- Open your usual email service in your browser or email app.
- Create a new message to info@centroevoluzionebambino.it. Use the service name as the subject.
- Attach the PDF you completed and saved. Check that your answers are visible, then send your message.
Please do not attach clinical records or identifying information about children. Any necessary documents and an appropriate sharing method will be agreed separately.
